ABSTRACT

Ex vivo lung perfusion (EVLP) could impact waitlist morbidity and mortality by increasing the number of transplantable allografts. Remote EVLP with a centralized lung evaluation system (CLES) at a dedicated facility has been shown to be feasible. There are no reports comparing the outcomes of remote vs local EVLP. Our institution has access to both modes of EVLP. Hereby, we describe the outcomes for remote EVLP (r-EVLP) and local EVLP (l-EVLP) at Mayo Clinic Florida. We did a retrospective analysis of the demographics, clinical characteristics, and outcomes of recipients of lungs that underwent EVLP as part of a r-EVLP clinical trial (NCT02234128) or at Lung Bioengineering Jacksonville (l-EVLP) with data obtained from the patient's electronic medical record. The r-EVLP cohort (n=10) tended to be younger than the l-RVLP cohort (n=12) (57.3 vs 61.6 years), and had a lower percentage of female recipients (20% vs 41.67% respectively). 80% of recipients were white in both cohorts. Most recipients were in the diagnosis group D (restrictive lung disease) in both cohorts. Three recipients in the l-EVLP group received a lung transplant due to complications from COVID-19. There were 5 single lung transplants (SLTx) in the r-EVLP (50%) and one in l-EVLP (8.33%). Lungs from donors after circulatory death (DCD) accounted for 40% of the allografts in the r-EVLP cohort and for 16.67% in the l-EVLP group. The median cold ischemia time (CIT) 1 was 5h:27min for the r-EVLP and 4h:35min for l-EVLP. The median CIT-2 time was 4h:16min for the r-EVLP and 3h:12min for the l-EVLP. EVLP time was similar for both groups. The median total preservation time was 13h:44min for the r-EVLP and 11h:38min for the l-EVLP cohorts. One (10%) in the r-EVLP and five (42%) in the l-EVLP groups were on ECMO at 72 hours post-transplant. Most of the remaining patients in both groups had a PGD-1 at 72 hours. All patients were alive at 30 days, and there was one death on each group at 1-year. At our center, survival at 1-year appeared similar in recipients of lungs assessed on r-EVLP or l-EVLP. Postoperative ECMO was used more frequently in the l-EVLP group. Median CIT-1 and CIT-2 were longer in the r-EVLP compared to the l-EVLP group by 52 and 64 minutes, respectively. Limitations of this study include single center retrospective experience, small sample size and lack of long-term outcomes. Future research comparing r-EVLP vs l-EVLP is warranted. ABSTRACT

Background Coronavirus disease 2019 (Covid-19) presents as a mild infection in up to 84% of cases out of which 10% progress to more severe disease requiring hospitalization and 33% to 69% progress to long haulers. The real-world data on the use of anti-severe acute respiratory syndrome coronavirus 2 spike neutralizing monoclonal antibodies, namely casirivimab and imdevimab, as well as bamlanivimab for mild Covid-19 is limited. Methods We conducted a retrospective cohort study on patients who received casirivimab and imdevimab or bamlanivimab for the treatment of mild Covid-19. Results Three hundred forty patients were included in the study with the mean age of the cohort 54.7 +/- 17.5 years, and 36.5% of them were men. Thirty-day outcomes revealed that 300 (88.2%) patients were followed up to 30 days, out of which 243 patients (71.5%) were asymptomatic at 30 days, 57 patients (16.7%) had symptoms at 30 days out of which 9 patients (2.6%) had an ED visit or were admitted because of the worsening of Covid-19 in 30 days and 3 patients (0.9%) died. Conclusions Our study suggests that early infusion with monoclonal antibodies may decrease the development of long haulers. Further studies with randomized-controlled trials are needed to understand more about monoclonal antibodies in the prevention of long-term sequelae especially in the setting of ongoing viral mutations.

ABSTRACT

COVID19 is the abbreviation used for Corona Virus Disease, is a communicable disease which is caused by a newly found Corona Virus. Most common indications are fever, dry cough, and tiredness. The outbreak was first identified in December 2019 at Wuhan City in the country China. On 30th January 2020, The World Health Organization (WHO) proclaimed the outbreak as a Public Health Emergency with International importance and duly announced it as a pandemic on March 11th2020.In India, the first case of Covid19, which emanated from China, was set forth on 30th January, 2020 at Thrissur District in Kerala. On March 24th, The Prime Minister made a decree of nationwide lockdown for 21days, to stop spreading of the pandemic, which in turn had a great impact on the life of 1.3 billion people in India. Kerala government has announced shut down of all schools and colleges in Kerala due to the outbreak. Due to Lock down for the last few months, every people got affected all over the country. There were lots of Socio-Economic problems emerged due to Lock Down with various groups of people around the country. Education Sector was highly affected because of the immediate close down of schools and colleges. Since the lockdown has announced during the month of March, students and teachers were deeply confused of conducting exams and all. In the next academic year schools and colleges were started functioning with Online Classes using different platforms like Google Meet, Microsoft Teams, Zoom etc. and Offline classes using WhatsApp, YouTube Channels etc. When teachers in Self-financing Colleges are concerned, they are less paid in terms of salary and other emoluments as compared with the teachers in government aided colleges. Most of them are working on contract basis and this brings job insecurity to them. When the colleges started with Online mode of teaching, they have to bear with the situation as same as the government aided teachers but with lesser benefits. Some may face financial crisis and some may have increased their workload. They also got some kind of benefits with Online teaching. Even though, Online teaching has got so much of importance and benefits now a days, the entire academic sessions conducted through Online mode become a tough task for the teachers and students. Data connectivity is considered to be an important factor for the successful conducting of teaching through Online Mode. Thus, this research work tries to find out the challenges faced and the advantages of online teaching of Self-financing College teachers in Thrissur District during pandemic period.

ABSTRACT

Novel Coronavirus disease (COVID-19) is an infectious disease caused by a newly discovered coronavirus. Most people who fall sick with this disease will experience moderate symptoms and recover without any special treatment but those who are having respiratory and cardio diseases may affect the infections in a serious way. The infection was seen in Kerala by the end of February, by the end of March Kerala Government declared lockdown for keeping the people more safe. Lockdown was started for 7 days and government extended it to end of April 2020. Gradually the government give relaxation on lockdown. Different sectors were affected in this pandemic followed by lockdown. Migrants employees were inattentive group of people those who were belongs to less privileged category moved from different parts of the country for their livelihood ,movement of people from the place of birth to anywhere to reside and settle can be called as 'migration'1. According to IOM2 "migration is the movement of a person or a group of persons either across an international boarder or within a country/state". The process of migration is as old as human history as it pave the way for forming different civilizations and surely, it will continue till human life exists. This study tries to analyze how the migrants face the financial issues in the time of lockdown 2020, it also tries to find out the socio-economic background of the in-migrant workers. Thus to explore the reasons behind their migration to Kerala and their working atmosphere, heterogeneity among them in terms of place of origin, age groups, culture, educational standards, their consumption pattern, improvements in earnings, savings, living standards etc. Therefore this study stands relevant and timely in the light of above mentioned dimensions. The economic conditions of the migrant workers in Ernakulam district at the time of lockdown, sample collected was 134 migrants analysis of statistical data we use SPSS 22 version. Used Excel for graphical representation of the data, mainly carryout statistical test such as: Paired t test& Cross tabulation.Descriptive design with quantitative method is used._

ABSTRACT

Breast cancer is the most common type of cancer affecting women. The formation of lumps in the breast is one of the first signs of the presence of this disease. These tumors can either be cancerous or benign and hence a breast tissue biopsy is conducted to determine their nature. Advancements in the field of vision-based Deep Learning have facilitated the wide adoption of automated diagnostic systems in hospitals, for tasks such as cancer and COVID detection from lung X-ray scans, diabetic retinopathy detection from retinal fundus images, brain MRI segmentation, etc. Moving forward, reduction in training, validation and development times, and efficient usage of training resources for these models will be more in focus. The EfficientNet architecture proposed by Google has recently outperformed prior state-of-the-art architectures such as DenseNet and ResNet on the ImageNet classification task while using fewer parameters and epochs to converge faster. In this paper, we compare the performance of the EfficientNetB3 architecture with the above-mentioned architectures for the tasks of binary and multinomial tumor classification on the benchmark BreakHis dataset, which consists of around 8000 breast histopathology images of varying magnification. Our results show that under similar training conditions, the EfficientNetB3 can converge faster and outperform the previous benchmark models by a significant margin. Our best models achieved 100% sensitivity and accuracy on certain binary classification tasks and a sensitivity of 95.45% and precision of 95.15% on 8-ary classification tasks. © 2020 IEEE.

ABSTRACT

"Purpose of education is to moralise and socialise the people," said Dr B. R. Ambedkar. The role of education in the holistic development of a society is immeasurable because good education shapes people right from their childhood and thus the entire society. That's why every developed civilisation had given keen importance to education in any form. The way people get educated is a reflection of the culture of that period. It has travelled through rock carvings, manuscripts, papers and today it has reached our smartphones due to the Covid'19 Pandemic situation. So, the researcher attempted to study the College Students Awareness about Online classes during the Covid'19 Pandemic situation.
